Why do we need health forms and immunizations records from our Doctor for all MA camps?
ALL MASSACHUSETTS CAMPS MUST COMPLY WITH REGULATIONS OF THE MASSACHUSETTSDEPT. OF PUBLIC HEALTH AND BE LICENSED BY THE LOCAL BOARD OF HEALTH).
A complete copy of the policy shall be furnished to parents upon their request.
